Incident Reports
INCIDENT REPORTS:
Please use this link to report an incident. All incidents will be reviewed by the Safety Committee.
Most reports can be Incidents which include 'Near Miss'.
SAFA member Lookup app. You can search for SAFA members by Full Name or partial SAFA# in order to check their membership and qualifications. If a member does not appear, it means they are not a financial member. If a qualification does not appear it probably means it has expired. This app will check for a new database version once per day and update as required when there is an internet connection. If your device has a camera and you have a membership card with a QR code on the back you can scan it to retrieve the details.
The incident reporting on this website should be consistent with the SAFA AIRS report.
Please read the AIRS User Guide attached prior to submitting a report so you know what is required.
The fundamental objective of accident and incident reporting is the prevention of accidents and incidents through shared learning. The review of accidents and incidents aims to determine the factors involved and use this information as a means of enhancing our levels of safety. The AIRS is the means of capturing this information.
Incident Report / Injury Report
All reports are made in the Incident System. If there is an Injury please complete an injury report and link the 2 reports. This must be done to comply with Privacy Laws. ALL injuries must be listed in the Injury Report. The incident report captures all other relevant information. The pilots must consent to Injury Details being shared. This is asked at registration and linked to their member profile.
Definition of Accident, Incident and Serious Injury
Incident: means an occurrence other than an accident, associated with the operation of the aircraft
including:
(a) hard landings or other events where there was potential for serious injury (including near misses), or
(b) use of rescue equipment or crews to extricate the Pilot and or aircraft from the landing site.
Accident: means a matter involving an aircraft where:
(a) any person dies or suffers serious injury as a result of an occurrence associated with the operation of the aircraft, or
(b) the aircraft incurs damage or structural failure that adversely affects the structural strength, performance or flight characteristics of the aircraft that would normally require major repair or replacement of the affected component(s), or
(c) any third-party property is destroyed or seriously damaged as a result of an occurrence associated with the operation of the aircraft, or
(d) the aircraft becomes lost or inaccessible.
NOTE: See Serious Injury definition.
Serious injury: is defined as any injury, other than fatal which:
(a) requires hospitalisation for more than 48 hours commencing within 7 days from the date the injuries were received.
(c) results in the fracture of any bone (except simple fractures of fingers, toes, or nose).
(d) involves lacerations which cause severe haemorrhage, nerve, muscle, or tendon damage.
(e) Involves injury to any internal organ.
(f) involves second- or third-degree burns, or any burns affecting more than five percent of the body surface.
